Insurance Policy

I do not accept insurance and maintain a direct pay practice. However, I’ll gladly provide insurance statements for anyone wishing to file a claim on their own with hopes of reimbursement. Please note: a diagnosis is required on these claims in order for them to be processed through your insurance company.
